The Afon Elai Partnership is a newly formed General Practice caring for 16,000 residents of South-West Cardiff. We are a merger of 2 longstanding and successful practices, Westway Surgery and Meddygfa Lansdowne Surgery with eight partners working from 2 sites easily accessible for the M4. We have an ethos of aiming to provide high-quality patient centered care, underpinned by a commitment to fairness and a focus on prioritising wellbeing over profit. We now have an exciting opportunity for a salaried GP to join our team, and help shape the future of a new strong and sustainable practice. Our balanced clinical team consists of GP partners, Salaried GPs, Physician Associates, Practice Pharmacist, Practice Nurses and Health Care Assistants alongside an excellent team of Patient Care Administrators providing care navigation and administrative support.
As a training practice we host registrars, medical students, general practice nurse trainees, foundation pharmacists and physician associate students and are committed to continuing professional development.
We deliver a full range of enhanced services facilitating the development of special interests and engage in non-commercial research but are looking to expand into also providing commercial research. We currently look after 6 local care homes, and are looking at expanding to take over a seventh, with the help of the right candidate!
We are a pro-active member of Cardiff South-West Cluster, with one of our GP partners previously taking the role of Cluster Lead Community Director, a position which is now filled by our previous practice manager who we continue to work closely with, and another GP Partner is Locality Director. We work with local partners to explore how best to work in a cooperative way to maximise our patients' wellbeing. In addition, our cluster has established weekly multi agency multi-disciplinary team meetings and an active hub delivering welfare checks and health monitoring. Job overview
The successful GP will be passionate about healthcare delivery and be keen to develop and work alongside our team at a busy and well-established practice of over 7,500 registered patients across two sites in Ferndale and Maerdy. Main duties of the job

De Winton Field Practice is a training practice situated in Tonypandy with a current list size of 9,350 patients and we are looking to expand our nursing team. We provide high quality primary care services in a modern health centre setting. The practice is located approximately 30 minutes from Cardiff with good transportation links. The practice is currently a Vision software practice but will be moving over to EMIS soon.
The Practice is looking for a Band 5/ 6 Practice Nurse, rate of pay will be dependent on level of experience and qualifications. Hours of work will be between 8.30am – 6.30pm. The working days and hours are negotiable for the right candidate. This is a permanent post after a successful probationary period.
Annual leave and Bank Holidays will be pro rata to the hours worked. The practice currently offers 5 weeks annual leave plus bank holidays whole time equivalent (WTE).

We are looking for a motivated, forward-thinking Pharmacist to join the multi-disciplinary team at St Andrews Surgery. The practice team consists of 3 GP partners, 1 Clinical Pharmacist, 3 Practice Nurses, 2 Health Care Assistants and a very supportive administration and reception team.
The role will be varied – answering medication queries; managing repeat prescription authorisations and re-authorisations; undertaking medicines reconciliation and various other aspects of medicines management. You will also provide prescribing advice and support to the practice clinical team which will include undertaking audits, developing protocols, providing feedback to prescribers, and monitoring improvements.
As part of your role, you will also support the practice team with chronic disease management, shared care and anticoagulation medication processes as well as supporting the medication processes within nursing homes.
We believe this to be an exciting opportunity for a pharmacist who would like to apply their skills to ensure safe and efficient prescribing in general practice. We would always welcome informal visits to meet the practice team.
