About the Practice
Cwm Garw Practice is a well-established dispensing GP practice serving a rural and semi-rural population in Wales. We provide comprehensive General Medical Services (GMS) and pride ourselves on delivering high-quality, patient-centred care within NHS Wales.
As a dispensing practice, we offer an integrated service to our patients, supported by an experienced and efficient dispensary team. We work closely with our local Primary Care Cluster, community services and secondary care colleagues.
Our multidisciplinary team includes GPs, practice nurses, pharmacists, HCAs, care coordinators, dispensary staff and a strong administrative and management team. We actively promote staff wellbeing, professional development and sustainable working practices.
The Role
We are seeking a motivated, dynamic and compassionate Salaried General Practitioner to join our friendly and supportive team. The post holder will deliver high-quality primary medical care in line with the GMS contract, NHS Wales policies and clinical governance requirements.
The role offers clinical autonomy, flexibility and the opportunity to work within a collaborative multidisciplinary environment.

An exciting opportunity has arisen to join our friendly and supportive team at St Isan Road Surgery for a fixed term to cover maternity leave. Our practice has been newly extended and refurbished and serves a population of approximately 8,000 patients. The team consists of 3 Partners and 2 Sessional GP’s, 3 practice nurses, 1 HCA and a full complement of highly experienced admin support staff.
We are situated in the Heath, a lovely part of Cardiff, only 3 miles away from the City Centre and close to excellent schools and parks.
This post would be a fantastic opportunity for any GP or ANP with an interest in, or experience of supporting students and teaching. A regular on call is associated with this post, but the candidate should be prepared to cover an on call during any period of absence/annual leave.

We are looking for a motivated, forward-thinking Pharmacist to join the multi-disciplinary team at St Andrews Surgery. The practice team consists of 3 GP partners, 1 Clinical Pharmacist, 3 Practice Nurses, 2 Health Care Assistants and a very supportive administration and reception team.
The role will be varied – answering medication queries; managing repeat prescription authorisations and re-authorisations; undertaking medicines reconciliation and various other aspects of medicines management. You will also provide prescribing advice and support to the practice clinical team which will include undertaking audits, developing protocols, providing feedback to prescribers, and monitoring improvements.
As part of your role, you will also support the practice team with chronic disease management, shared care and anticoagulation medication processes as well as supporting the medication processes within nursing homes.
We believe this to be an exciting opportunity for a pharmacist who would like to apply their skills to ensure safe and efficient prescribing in general practice. We would always welcome informal visits to meet the practice team.

Job Summary
The post holder will lead and support expert clinical practice, education skills, leadership and role development, be expected to exercise a high degree of professional autonomy and make critical decisions to satisfy the expectations and demands of the role.
The post holder will facilitate the development of clinical practice and take a lead role in the clinical assessment of patients who have a complex and undetermined presentation. They will work alongside Medical and Pharmacy colleagues to maintain patients in a nursing home setting and will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.
The post holder will act as an expert resource within the Practice team, providing specialist advice regarding the clinical management of individuals presenting with minor illness or chronic conditions.
