We are looking for salaried GP, working Monday, Tuesday and Wednesday, to join a welcoming team of six partners.

We aim to provide a supportive working environment, with annual reviews, mentoring available if requested, regular social activities, and a fair and evenly distributed workload within the practice team.  Salaried GPs are not required to summarise any Docman Workflow letters or new patient notes. The working template can be flexible, with a mix of routine and emergency appointments. We would also expect the GP to do home visits when required and there will be a rota’d on call day also.

The successful candidate will provide 6 clinical sessions per week. We can offer a degree of flexibility in start/finish times where needed to accommodate other commitments such as childcare etc.

- Practice list size 10,835

- 6 weeks annual leave.

- Salary depending on level of experience. MPS fees included.

Four Elms Medical Centre is seeking a highly motivated and compassionate Salaried GP to join our friendly / Supportive and forward-thinking team. This post is for 6 sessions a week ( 3 Days ) working across both our Newport and Cardiff sites.

We are a well-established, forward-thinking practice supporting a 25,000 patient list shared over three sites. One site located in the Cardiff Royal infirmary, Another in Pengam Green Cardiff and also in Ringland, Newport. Job Summary

The candidate will work in a large, busy, modern GP Practice with a list size of 25,000 patients. The post holder will join a team of four Practice Nurses, four Health Care Support Workers and two phlebotomists providing clinical support in the areas of diabetes, respiratory, women’s health and family planning, wounds, among others.

Duties/Responsibilities

The post-holder will undertake a range of duties and responsibilities as part of a team to support the Practice in delivering care to its patients. Main duties and responsibilities include but are not limited to:

Chronic disease management to includes diabetes, respiratory and women’s health as examples

Immunisations

Primary Care Pharmacy Technician

Job Purpose and Main Duties

The Primary Care Pharmacy Technician will work alongside the practice Clinical Pharmacist to support the practice clinical team in all aspects of prescribing and medicines safety.

The purpose of this role is to support improvements to maximise safe and best practice in prescribing and improve the quality of patient care. You will be a point of contact for patient’s enquiries regarding their prescriptions. The post holder will help patients to get the best from their medicines through effective medicines reconciliation after discharge and on other transfers of care, identification of patients potentially suitable for repeat dispensing, providing assistance in the monitoring of high-risk drugs, and handling of prescription queries within the areas of competence.

The role will also include training and development of practice based prescribing clerks and administrators to support improved co-ordination and effective pharmacy administration within general practice, especially with relation to repeat prescribing systems and processes.

Job Duties

·        Support to the practice Clinical Pharmacist and practice Clinical Team to provide a safe and effective repeat prescribing service

·        Comply with legal and professional requirements and with safe systems of work for re-authorising repeat prescriptions.

·        Co-ordinate medication review appointments and any changes of medication as advised by the clinical team

·        Liaise with pharmacies on the supply of prescribed drugs, alert clinicians of any supply issues and forward recommendations of alternatives

·        Medicines reconciliation post discharge and on other transfers of care.

·        Bring queries to clinical team to include under or over usage.

·        Day to day handling of prescription queries within area of own competence.

·        Delivery of training and development to practice based prescribing clerks and administrators to support improved co-ordination and effective pharmacy administration within General Practice.

·        Use and maintain information reporting systems and databases relevant to the position including incident reporting, prescribing data and patient outcomes.

·        Participate in practice meetings, and other meetings to improve engagement of the role and to promote issues relevant to prescribing.

·        Liaise with multidisciplinary team, prescribing support team, community pharmacy and hospital pharmacy teams.

·        Handle function specific information, which may be sensitive, complex or confidential and appropriately recording, transferring and/or coordinating such information in accordance with the GDPR; Caldecott Guidelines and the Confidentiality Code of Conduct.

·        Be responsible for the organisation, planning and of own workload to meet set deadlines.

·        Utilise GP practice clinical systems to conduct prescribing audits.

·        Support the Clinical Pharmacist with the work required under the UHB Medicines Management Incentive Scheme.

·        Support the Clinical Pharmacist in Share Care monitoring and processes.

·        In collaboration with practice Clinical Pharmacist and GP Prescribing Lead ensure that all major MHRA drug alerts, where appropriate and within levels of competence, are acted upon within practices over an appropriate time frame and to advise on any medication changes that occur as a result

·        Undertake training and development, to ensure the required knowledge and skills for practice based work

·        Escalate problems or issues to line manager that requires additional support or input

·        Carry out other duties which are appropriate to the skills and competencies of the post holder and grade of the post as the priorities of the service change

·        Establish and maintain effective working relationships with GP practices, staff and other healthcare professionals

·        The post holder will: Develop a culture that promotes equality and values diversity. We are looking to recruit an enthusiastic and committed call handler to join our friendly team. We have three part time 30-hour posts Monday-Friday. We are ideally looking for someone with excellent interpersonal skills to deliver high quality services to our patients.

Duties will include general assistance to the practice team, answering the telephone, care navigation, sign posting patients to the most appropriate service or healthcare professional in a courteous, efficient and effect way.

Main Duties of the job

•Answering incoming calls, transferring calls or dealing with callers requests appropriately.
•Signposting patients to the appropriate service or health care professional in a courteous, efficient and effective way.
•Offer general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone.
•You will be working between the hours of 8am and 6.30pm there may be a need to work at our branch sites.

Experience and personal specification

•Call handling/dealing with the public experience working in healthcare is not essential.
•Excellent communication skills.
